Our Miracle Child Grant eligibility depends on many considerations including the underlying cause of infertility, financial situation, and other life factors as well as meeting the eligibility requirements listed below. Our Miracle Child does not reimburse for treatment already begun.

We value diversity in regard to race, ethnicity, sexual orientation, and geography. Grants are available to same-sex and heterosexual married couples who are permanent residents of the United States of America and fit the infertility criteria. This grant is meant for those who don’t have sufficient insurance coverage or funds to afford infertility-related treatment/medication on their own.

Meeting grant requirements does not assure receiving a grant. We understand that there are many applicants who need financial assistance for infertility treatment, but we are unable to award a grant to more than one couple per application cycle as of right now.

Eligibility Checklist

Type of treatments/procedures covered: Our Miracle Child funds a range of procedures including (but not limited to) egg and sperm donation, in vitro fertilization, embryo donation, and gestational surrogacy.
Failure to conceive naturally: The Our Miracle Child grant is designated for patients with a medical diagnosis of infertility. In general, infertility is defined as not being able to get pregnant (conceive) after one year or longer of unprotected sex.
Residence: Applicants must be permanent residents of the United States.
Treatment: Applicants must be under the care of an accredited fertility facility (of their choice) within the United States.
Application: Please only submit one application per grant application cycle.
Receiving financial aid from other charities: The Our Miracle Child grant is not meant to cover full treatment or medication costs. We strongly encourage you to apply to other charities in addition to the Our Miracle Child Foundation in pursuit of procuring sufficient funding for your fertility treatment. Your fertility timeline: We ask that the grant money be used within 12 months of the grant recipient announcement date, unless other arrangements have been made among the Foundation, the applicants, and their physician.

The Application Process


What you need to know

  • Application submission deadline: late August/early September
  • You will be required to print and complete the Grant Agreement Form, Media Release Form, and Medical Evaluation Form then upload them into their designated sections of the application. Please email us if other arrangements need to be made to submit these documents.
  • If you fill out the application form using a free Google account while online, your work will be autosaved. Learn more about the autosave feature here.
  • Applications MUST be received by the Foundation by the deadline. Applicants may be contacted to provide additional information. All applicants will be notified via email when their application has been received.

What you need to include with your application

Complete, in its entirety, the Our Miracle Child Grant Application Form. Here is a preview version of the form for your convenience (DO NOT fill out, only for reference).

NOTE: The sections of the application that are required are marked by red asterisks. Please only answer the unrequired questions that apply to you. Although the preview version looks long, the required questions make up only a small portion of the application. Answer unrequired questions ONLY if they apply to you.

Application Form: Please submit this entire application form, completed by the deadline. Personal Story: Use this space to tell us about yourself – outlining hobbies, profession, family history, and anything else that may help us learn more about you! Do NOT document your fertility history here. Maximum 2 pages, preferably typed.
Photo(s): Include at least one photo of both of the applicants.
Grant Agreement Form: All information submitted to Our Miracle Child will be held in strictest confidence and viewed only by the selection committee.
Media Release Form: The personal story and/or photo(s) of the grant recipients may be shared by the Foundation according to the guidelines of the Media Release Form. Recipients may be contacted after receiving the grant to share a personal statement for the Our Miracle Child Foundation website and social media platforms as well as participate in upcoming events. Only the first names and location (state) of the recipients will be shared on the Our Miracle Child website and social media platforms. If you have any questions or concerns, email us and other arrangements can be made.
Medical Evaluation: Your physician MUST complete the medical portion of the application. It is the applicant’s responsibility to obtain these pages from their physician. Note that fertility clinics and physicians may require some time to get the papers back to you. Note: If using a surrogate, the medical evaluation form must be completed for the surrogate. If the surrogate is provided by an agency, the agency must provide proof of medical clearance.

What happens after you submit your application?

Once you submit your application, you will receive an email confirmation with a copy of your responses. Some applicants may be asked to provide additional information. We must receive your application before the deadline for it to be considered.

After submitting your application by the deadline, selected recipients will be announced on the website and social media platforms on a TBD date. The grant recipients will also receive an email confirmation after the announcement.
